HVA is seeking nominations for our annual Emergency Medical Services Recognition Awards.  Nominations are due Friday, April 8, 2005.  Click here for a nomination form.  The Awards are as follows:

Certificate of Merit Award (internal) - For HVA employees throughout the year who do a particularly good job on a specific project or call.

Lifetime Achievement Award (internal or external) - For an HVA employee, HVA Board member or other community leader who made a long-term, outstanding contribution to HVA and/or the EMS industry in our service area.

Retirement Recognition Award (retiring HVA Board members and employees) - For an employee who has retired from HVA with a minimum of 15 years of service, or to a retiring Board member who has completed at least one full term in office.

Life Saving Award (internal or external) - For an HVA employee or citizen who has saved, or attempted to save a life under circum-stances that indicate heroism and risk to his or her own life.

Special Recognition Award (external) - For someone who has demonstrated leadership in the advancement of EMS in Michigan or nationally, or for other significant reasons as approved by the HVA Board of Trustees.

Award winners (including the HVA 2004 Professional Achievement Award winners) will be honored during an Awards Dinner on Wednesday, May 18, 2005 at 6:30 p.m. at the Morris Lawrence Building, Washtenaw Community College, 4800 East Huron River Drive, Ann Arbor, Michigan.
